Manufacturers must report the following events in accordance with ICAO Annex 8:
 
a) fires caused by failure, malfunction or improper operation

b) failure, defect or malfunction of the engine output system which causes damage to the engine, adjacent aircraft structure or its equipment
 
c) accumulation or circulation of toxic, or harmful gases in the flight deck or passenger compartment
 
d) improper operation, failure or malfunction of the propeller control system
 
e) failure of the structure of the propeller, propeller blade or rotor head
 
(f) leakage of flammable liquid in areas where ignition sources are likely to be expected
 
g) failure of the braking system caused by failure of the structure or material during operation
 
h) significant primary defect or failure of the aircraft structure caused by fatigue, insufficient strength, corrosion, etc.
 
i) any, unusual vibrations or jitter, caused by a failure or defect of the structure, or improper operation, malfunction or defect of the system
 
j) engine structure failure and in-flight engine stop
 
k) failure, malfunction or improper operation of any aircraft design or control system which prevents the normal control of the aircraft or impairs its flight characteristics
 
l) complete loss of two systems of electrical or hydraulic power sources during one flight of the aircraft
 
m) failure or defect of more than one instrument indicating flight position, altitude or speed during a single flight
